Ohio diver's death blamed on company safety violations

U.S. Aqua Vac Inc. is an out-of-state company that provides diving services in Ohio and nationwide. Divers help to clear debris from lakes and ponds, as was the case last November when a company diver entered a pond in New Albany. The 23-year-old man died after he ran out of air while working.

Safety violations can make Illinois workplaces dangerous

The Los Angeles District Attorney's office has filed charges against Bumble Bee Foods and two of its managers regarding the fatality of a 62-year-old employee on Oct. 11, 2012. According to officials, the charges involve the willful violation of safety regulations at the workplace. The company could owe over $1 million dollars while the two accused men could face up to three years in prison and could be made to pay up to $250,000 in fines.

Construction workers die in accident involving mast climber

Illinois residents might be interested to learn about a fatal accident involving construction workers that took place in North Carolina on March 24. While they were working on the Charter Square building in downtown Raleigh, three workers fell to their deaths and a fourth worker was seriously injured. All of the men who were involved in the accident were Hispanic, and they ranged in age from 33 to 53.

Worker crushed to death after getting stuck in the snow

A worker in Illinois was killed on Feb. 13 after he was crushed between a tractor and a container. The 22-year-old man had been working for CenterPoint Intermodal Centers in Joliet when he died. Following the fatal accident, representatives of the federal Occupational Safety and Health Administration began an investigation to look into the causes.

One killed, two injured at chemical plant

Illinois workers may have heard that a chemical corporation was cited for 17 health and safety violations by OSHA after one employee was killed and two others were injured. In 2012, the company also received 19 citations after an OSHA inspection. The director of an OSHA regional office described the repeated violations as displaying a lack of commitment to employee health and safety. The employee died after his respiratory system was burned from hazardous chemical vapors released by a reactor that was over pressurized.

Illinois woman accidentally killed at work in thrift store

A 54-year-old wife and mother of two who had been working for a thrift store in Washington Heights for more than 25 years lost her life to an accidental shooting on March 7. She was sorting through some donated clothing items with a male co-worker as part of her job at the South Ashland Avenue facility. Inside a sock, the co-worker found a .22 caliber handgun that, when the man shook it out of the sock, accidentally discharged, hitting the woman in the chest.
